Going off how the NCLEX adapts, I don't necessarily think your pass or fail was entirely dependent on that last question. I mean it certainly could've been, just not the case 100% of the time that someone goes to 145. Plus keep in mind, some people are randomly selected to go all the way to 145 questions, no matter how well you're actually doing. I got the bad pop up! So I took my NCLEX today and finished at 77 questions. After the exam I did the trick and got the bad pop up. During the exam, I felt like I was getting easier questions and every time I would get a SATA question, I would then get a multiple choice question. I only had like about two in a row and then back to multiple choice. I cried A LOT and was pretty sure I failed. I did the Pearson Vue trick no less than 1000 times and got the "bad pop up" every time. I read post after post about how the trick is supposedly 99.9% accurate which made me even more depressed.
